Ohio EdChoice Voucher Program

If you are enrolling a new student in grades K-4, in addition to the children currently attending St. Lawrence, you must bring the forms mentioned below to the school office along with the child’s original birth certificate.                         

Private Pay Grades K-7: 

1. Complete the pink re-registration form in today’s parent communication envelope.                                                         

2. Send registration form with fees.        

   1. $25.00 family cafeteria fee.     

   2. $50 per student registration fee.

EdChoice Grades K-7:

1. Complete the pink re-registration form in today's parent communication envelope.

2. Complete the EdChoice renewal form.  (Received by mail from the State of Ohio.)

3. Send both forms to the school office with the following:

   1. $25.00 family cafeteria fee

   2. Proof of residence.  (Current utility bill in your name or letter from Hamilton County Job & Family Services.)

Returning Preschool: 

1. Complete the white pre-school registration in your parent communication envelope.

2. Send registration form with $50 registration fee to the office.

Kindergarten/Preschool:   If you have a new kindergarten or preschool student, you will need to come to the school office between 9:00 am and 2:00 pm to register.  You will need to bring his/her original birth certificate and social security card to the office to be copied.  If you are enrolling a kindergartener under the EdChoice Scholarship program, you must also provide a current proof of residence.  Kindergarteners must be 5 years old by September 30, 2012.  Preschoolers must be 3 years old by September 30, 2012 and must be potty trained.  Kindergarteners will be signed up for a time to come to the kindergarten screening on May 8th.

St. Lawrence School Policy Concerning Refunds on Student Tuition/School Fees

At the time of registration, initial tuition and cafeteria fees for each student must be paid in full. 

PLEASE NOTE:

IF A REGISTERED CHILD WITHDRAWS BEFORE THE BEGINNING OF THE NEW SCHOOL YEAR, OUR ST. LAWRENCE POLICY IS AS FOLLOWS:

• AFTER REGISTRATION AND PRIOR TO THE FIRST DAY OF SCHOOL:

  • The cafeteria fee is refunded.  The initial registration fee is not refunded.

   

• BEGINNING WITH THE FIRST DAY OF THE SCHOOL YEAR

  • NO refunds for cafeteria fee or registration/tuition fees/payments.
